Several months ago, a breach of security occurred within Ivy Rehab, a New York-based chain of about 200 outpatient physical therapy clinics, including 35 in Virginia. In Hampton Roads, they operate as Southeastern Physical Therapy. An article published on the Virginian Pilot today mentioned that our physical therapy clinic is part of their system. We do contract with them to provide physical therapy services; however, our systems are completely separate, so none of our patients' information has been compromised.
Patients who think their information may be at risk can call 833-935-1376 from 9 a.m. to 9 p.m. Monday through Friday. Ivy Rehab is offering free identity theft restoration and credit monitoring through Equifax to patients who were affected.
